The ageing process is associated with thinning and dryness of the skin caused by loss of collagen and elastin fibres, bone atrophy and a loss of facial fat, resulting in a tired look, superficial lines and wrinkles, lowering of eyebrows, droopy eyelids, deepening of tear trough and nasolabial folds, loss of volume in cheeks, “smoker’s lines”, narrow lips and loss of jawline definition. Carefully applied selected fillers can treat all of these conditions.
INTRODUCTION
Most commonly used dermal fillers are made of cross linked hyaluronic acid (Juvederm, Restylene, Belotero, Teosyal, MaiLi). Other type of dermal filler are biocompatible, reabsorbable, synthetic substances (Ellanse - policaprolactone, Radiesse - calcium hydroxyapatite, Sculptra aesthetic - poly-L-lactic acid). They are characteirised by less water absorbtion and more collagen stymulating properties giving longer lasting and natural results. A hybrid type of dermal fillers combine hyaluronic acid with calcium hydroxyapatite +/- aminoacids to enhance their properties ( HArmonyCA, Neauvia Organic) Applications of dermal fillers differ depending on the type of the products and its qualities. Some are used to diminish the appearance of fine lines and wrinkles on forehead, crow’s feet or “smoker’s lines”. Others help to lift the skin reducing folds such as nasolabial folds or Marionette’s lines. Some are used for deeper volume replacement in temples or cheeks whilst others are used for facial contouring for example cheeks and jawline. The longevity of dermal fillers ranges from 6, 12, 18 to 24 months following which they become reabsorbed and eliminated from the body. Whilst in the tissue they have an ability to stimulate production of collagen meaning that maintenance treatment may require less of the product to be injected next time. Synthetic dermal fillers such as Ellanse or Radiasse remain in the tissue for 24 months allowing for fewer treatments, more collagen stimulation and tissue regeneration compared with hyaluronic acid dermal fillers. Ellanse has been shown to improve facial features over time resulting in only 50% reduction of effect after 2 years compared with hyaluronic acid dermal fillers. Injections are carried out with a sterile needle or a cannula using special injection techniques, including MD codes described by Mauricio De Mayo a Brazilian plastic surgeon. The procedure is generally pain-free, although some minimal discomfort may occur. Local numbing cream can be used 30 minutes before the procedure. Dermal fillers are reabsorb-able, soft implants used to diminish signs of ageing such as: wrinkles (around eyes or mouth), folds ( nose to mouth), add volume (cheeks) and define facial contour (chin, jawline). Other indications include under eye shadows, tear trough, or making hands appear more youthful. They can be used for “beautification” when minimal intervention results in an improvement of genetically predetermined imperfections such as underdeveloped chin or irregularities of the shape of the nose. Some dermal fillers play a role in managing vaginal dryness or improving vulval appearance through the Labial puff procedure.

HOW IT WORKS
Dermal fillers are commonly used to diminish the appearance of the signs of ageing on the face, neck, décolletage and hands as well as intimate parts of the body (Labial puff).
They are also used for “beautification” when minimal interventions result in an improvement in facial features, resulting in a more attractive appearance.
THE PROCEDURE
Injections are carried out with a sterile needle or a cannula using special injection techniques, including MD codes described by Mauricio De Mayo a Brazilian plastic surgeon.
The procedure is generally pain-free, although some minimal discomfort may occur. Local numbing cream can be used 30 minutes before the procedure.
Skin boosters (Profhilo, Restylene Vital Light, Volite, Redensity 1, among others) for skin hydration, reduction of fine lines and skin revitalisation. Usually, a course of 2-3 treatments is required, followed by maintenance.
RESULTS
